2. Ciprofloxacin 400mg IV Q12H +

Gentamicin/ Tobramycin 3-5mg/kg/day IV OD/ Amikacin 15mg/kg/day IV OD + Metronidazole 500mg IV Q8H Or

3. Amoxyclav 1.2gm IV Q8H +
