Relatives of patient, brought him to clinic. With complaints of patient unable to recognize them, drowsy, fever, myalgia, black scab. Admitted to ICU.

On examination GCS 11/15. Eschar scratched. Temperature-104 F, pulse 125BPM, respiratory rate 23 per minute, Spo2 97%.

Cardiovascular, respiratory and GIT examination was normal, patient was drowsy.

SGOT- 458, SGPT- 584, creatinine 1.8, WBC- 18,564 (predominantly PMN). IgM scrub positive.

Diagnosed with Scrub Typhus.

Scrub Typhus Treatment

Treatment started with I/V fluids, C. DOXYCYCLIN, URSODEOXYCHOLIC ACID, SILYMARIN and supportive care.

Patient improved within 3 hours. Discharged on day 7. And doing well now.
