Influenza (an orthomyxovirus)- Transmited primarily by droplet nuclei rather than direct contact or fomites (cloths, skin, hairs, etc).

Highly contagious.

Three types-

Type A infects Humans, swine, horses, birds, etc. (more severe disease)

Type B & C only infect humans

(causes less severe disease).

Subtypes: Swine Flu- H1N1 (same features like seasonal flu), Avian or Bird Flu- H5N1,

Only type A influenza have subtypes & caused Pandemic, currently circulating in humans are H1N1 & H3N2

1918 Pandemic caused by H1N1 Spanish Influenza.

2009 Pandemic caused by H1N1 Swine (pig) origin Influenza.

2013 H7N9 outbreak in China.

Type B has no subtypes.

Type C causes mild illness.

Type D primarily affect cattle, not in humans

Avian Influenza Or Bird flu H5N1:

Zoonotic- influenza A virus

Infection by infected dead or live poultry

Aggressive clinical course

Fever, cough, dysnoea, & gastrointestinal symptoms, etc

RT-PCR & Rapid antigen assay is available

No vaccine avilable for avian influenza for public

Avoid exposure is best, No risk of infection by well-

cooked poultry product (like chicken, egg)

Postexposure prophylaxis or

Treatment with C. Oseltamivir 75 mg BD

for 5 to 10days, start within 48 hours of exposure.

Influenza Cause:

Influenza (an orthomyxovirus) virus- Transmited primarily by droplet nuclei.

Influenza Symptoms:

Incubation period- 1–4 days

Fever (1-7 days), chills,  malaise, dry cough, nose stuffiness, flushed face, headache, nausea, myalgia, rhinorrhea, congestion, symptoms of pharyngitis, substernal soreness, hoarseness, etc.

Influenza Signs:

Conjunctival redness, pharyngeal injection,

fever more than 38.2 degrees celsius or 100.6 degree fahrenheit,

cervical lymph node enlargement, tracheal tenderness.

chest clear in uncomplicated, but rhonchi,

wheezes & scattered rales are reported in different outbreakes

Influenza Investigation:

Rapid diagnostic tests- detect influenza antigen from throat or nasal swab.

Rapid nucleic acid amplification & digital immunoassays- more sensitive

RT-PCR, virus isolation, leukopenia, proteinuria

Influenza Complications:

More in previously diseased person.  

acute sinusitis, purulent bronchitis, otitis media, pneumonia.

viral pneumonitis, severe hypoxemia, sometimes shock.

Reye syndrome causes rapid progressive hepatic failure & encepalopathy.

Influenza Prevention:

Annual administration- influenza vaccine- reduces hospitalizations by 61%.

Chemoprophylaxis for  influenza A &B- Capsule oseltamivir 75 mg daily, or 

Inhaled zanamivir 10 mg daily for 7 days after last exposure.

Influenza Treatment:

Capsule Oseltamivir 75 mg twice a day for 5 days

Inhaled Zanamivir 10mg inhalation twice a day for 5 days or

Intravenous Peramivir 600 mg stat

Laninamivir

Baloxavir

Dapivirine

complicated disease- oseltamivir,  ribavirin, & amantadine.

Influenza Prognosis:

Uncomplicated cases recover iv 1–7 days. 

Excellent prognosis in healthy, young adult (4-8% mortality).

10-15% mortality in pandemics & immunocompromised.
