Heart Attack TOMBSTONING STEMI, case discussion
35 year old male came in clinic with chest pain and he was drowsy. he was chronic heavy smoker. He was unable to understand anything. ECG showed, TOMBSTONING STEMI STEMI EXTENSIVE ANTERIOR WALL MI that means major heart attack. But he was suffering from CARDIOGENIC SHOCK. he was poor patient.
Treatment- THROMBOLYSED him with STREPTOKINASE. and We able to manage BP with dopamine.
Started with Enoxaparin, Ecosprin, Clopidogrel, Atorvastatin, Etc. No complications.
so he was asymptomatic after 2 hours.
CAG- angiogram done & revealed RECANALISED LAD. EF was 40%. Because of early and accurate diagnosis we able to save him. patient was discharged after 5 days and Now he is in good health.
